1. Please read and become familiar with Howlabaloo Dog Park Guidelines and Park Tips.

2. Download, print, complete and sign appropriate application forms and waiver. Complete information for each additional dog.

3. Bring the completed, signed forms, along with a copy of current veterinary vaccination records for each dog or you may have the veterinarian fax the forms to 814.734.1664. If your dog is spayed or neuter please bring proof.

Current:

1. Rabies, DHLPP, Bordetella
2. Negative Fecal and
3. Dog License

*Bordetella, leptospirosis, and a fecal test may not be given routinely by your veterinarian.  Please request these procedures.

Recommendations: Due to the environment of the park, we highly recommend that dogs be on a flea, tick, lyme, and canine influenza preventative program. We strongly encourage you to speak with your veterinarian for the best preventive treatment plan for your dog.

• You are responsible to keep your pet's requirements current. Any canine 'out-of-date' on any  requirement will be unable to use the park.

• Current rabies and dog license tags are to be worn at ALL times. If you are from outside of Pennsylvania and the county in which you reside does not require licensing, you must provide a letter on county letterhead stating "no licensing required."
